Course Description
Documentation is where professional thinking becomes accountable.
Therapeutic records are more than an administrative requirement. They reflect our clinical thinking, ethical judgement and professional responsibility.
This interactive two-day workshop explores the clinical, ethical and legal dimensions of therapeutic documentation, supporting counsellors, psychotherapists and clinical supervisors in developing greater competence, confidence and compliance in their record-keeping practices.
Through practical exercises, case studies and reflective discussion, participants will explore:
The clinical purpose of records and notes, and what constitutes meaningful documentation.
What to record, how much to record, and how to balance clinical relevance with data minimisation.
Writing clear, proportionate and professionally accountable therapy notes.
Ethical responsibilities, confidentiality and professional boundaries.
GDPR, data protection, retention and client access to records.
Documenting risk, safeguarding concerns and clinical decision-making.
Responding to Subject Access Requests and legal disclosure.
Records and notes within clinical supervision.
Common documentation dilemmas and the difference between clinically meaningful and defensive notes.
The programme integrates theory, professional standards and practical application, encouraging participants to reflect on their existing practices and develop a more confident, consistent and clinically grounded approach to documentation.
